Please fill out one form for each child preparing for a sacrament.

Every family preparing a child for a sacrament must also fill out a family registration form.

Prerequisites

First Penance prerequisites

child is at least 7 years old
child must have completed a minimum of 20 hours of catechesis in the previous year
child recognizes the difference between right and wrong

First Eucharist prerequisites

child is at least 7 years old
child must have completed a minimum of 20 hours of catechesis in the previous year
child has received the sacrament of First Penance

Confirmation Prerequisites

child is at least 11 years old
child must have completed a minimum of 40 hours of catechesis in the previous two years
child has received the sacrament of First Penance
child has received the sacrament of First Eucharist

Child meets all the prerequisites for the preparation of the sacrament.
